I have been reviewing my old English conversation textbooks. I have three questions.

(Exercise 1-5) Why are some people (always / every time) late?

The correct answer is "always", but why the expression "every time" is not correct?


(Exercise 2-6) He could have caused an accident.
What does sentence mean? Did he cause an accident?


(General question) In my textbook there are "annoy" and "bother". Do you know any other word which can be used when talk about pet peeves? Could you write example sentence(s), too?
