Though, in regard to 'a foreign language,' it depends on which one it is. For a western language, you could probably just read right away where as Asian languages you need to learn a new script or many characters as a first hurdle.

今、京都市に日本語学校で勉強しています。授業中、先生達と私達は日本語しか話せません。日本に3ヶ月住んでいたのに、私の日本語が早く上手になったと思います。チャンスがあったら、留学した方がいいです。
This is not exactly an efficient method, but I learn mainly by reading websites, short stories, papers, and blogs. Depending on my mood, I look up unfamiliar words on the spot, sometimes I don't.
Sometimes, I go on a study phase where I focus on a particular topic, ex. four-character compounts (四字熟語). I don't encounter them a lot while reading, but over time, they accumulate and start bothering me enough that I feel the need to study vocabulary blocks like that separately.
I try to learn as much vocabulary and collocations (chunks) as possible.
I use a grammar book as well.
I also have a lot of friends who speak the languages.
With some languages I listen to music.
