Mam 17 pięć lat i pochodzę ze Stanów Zjednoczonych.
1) I'm not sure what you meant by '17 pięć lat'. If you meant that you are 17 and a half than you should have said '17 i pół'. If you meant 17 years and 5 months old then it would be '17 lat i pięć miesięcy'. If it is just a typo then forget it.
2) Polish language has 7 cases' inflection, so when you say that you are from the USA you must inflect 'Stany Zjednoczone' to 'Stanów Zjednoczonych'. - It is the hardest part of learning Polish but don't be discourage!

"Z USA" is easier to use because you don't have to decline it. :) But remember that it's pronounced "OO-ES-A(h)"!
