The start of April, my husbund wake me up about 3:00am.
He said "Take me to a hospital."
He never go to hospital until he feels really sick.
At the hospital, the doctor immediately thought he had a heart attack.
There were many medical tests such as an electrocardiogram, a CT, blood test and other things.
The results showed there was no problem with his heart.
But, he had the same symptom again and again for 3 weeks and we went to four hospitals to find out what was wrong.
In the end, we went to a mental hospital.
The doctor gave him a check sheet which shows what the person's character like.
Then, we talk to the doctor.
He listened carefully.
Finally, he made a diagnosis which were autonomic imbalance and Panic disorder.
We've never thought of such things, and at the same time we were really amazed how one's mental effects one's body!

I think you mean checklist instead of check sheet but I'm not sure. The phrase "what the person's character like" is not clear to me.
