Culturally, I have been taught that first cousins are like your siblings. So while it is only strictly forbidden to marry your siblings, it is just rare to marry your first cousin. I think that it is unusual, but not "wrong".
I think there is a law against marrying your siblings in America, but none against marrying cousins. The minimum age in America for marriage is 18 years.
In my country, marrying your relatives is not very common. Whether they marry someone of the same or different nationality varies based on the individual.

Most university students at Leeds think marring your cousin is wrong (just another suggestion)

15-20, 21-25, 26-30 31-35 36-40 (you forgot one age group ^_~)
I believe each state makes its own marriage laws in America. In Florida minors who are 16 or 17 can get married if they obtain permission from their parents. From age 18 and up, you don't need your parent's permission.
Marrying your first cousin is only allowed in about 25 states, but prohibited in 25 (but some of these states make exceptions). Florida allows cousins to get married. Marrying your second cousin is allowed in all 50 states.
Apparently there is a concern about genetics and worry for children who might be born.
Punishments ... each state's law is different but basically you can be fined and/or put in prison for a while. Also the marriage will be considered void.
As for your final set of questions ... of course not everyone does this but I've noticed many people marry within similar race or culture, and also people don't usually marry in their family in Florida.
